The Purpose of Screening Forms
Screening forms are used during an intake process in which correctional staff triage those who may be at significant risk for suicide. Experience has shown that intake screening is a part of the overall procedure that can practically eliminate custodial suicides.
Intake screening is the first step and is crucial to determine whether inmates require a more specialized mental health assessment. Inmates will not receive treatment for their mental health if it is not identified as a need.
The screening form should be completed by a trained booking jailer or mental /medical health personnel.
The goal of providing information on this website is to help Texas inmates who are now or have previously been prisoners in county or municipal jails in the state. Making allegations that entities or persons have been involved in wrongdoing is never an intention.
