Following an inspection of Tyler County Jail in Woodville, Texas, the Texas Commission on Jail Standards (TCJS) released a Jail Inspection Report dated April 18, 2018. Some of the alleged violations of minimum jail standards put prisoners at risk and one puts prisoner, staff members, and the public at risk, as well.

According to investigators with TCJS, facility security requirements have been violated at Tyler County Jail. The planning of facility security is supposed to provide protections of prisoners from one another, deter or prevent escapes, and protect visitors and staff from prisoners. Members of the jail staff were observed in the booking area of the jail retrieving facility keys. A jailer made a statement in this regard, saying the keys were sometimes kept in the booking facility drawer for lack of a better place. The secured facility was left at risk because of this action.

During intake screening, prisoners who are known or observed to be potentially suicidal or mentally disabled are to be identified. In keeping with the Code of Criminal Procedures Article 16.22, the magistrate is supposed to be notified about such individuals, making it possible for referrals to be made to available mental health officials. In regard to this requirement, investigators allegedly found that the magistrates are not always being notified. Also, the supervisor was not signing the bottom of the forms, which is required to complete the process.

Required face-to-face observations of prisoners at least once every 60 minutes have allegedly continuously been exceeded by 1 to 3 minutes.
