50-Year-Old David Martinez is Fatally Struck by a Forklift in Laredo TX

Forklifts create workplace hazards that can be fatal and, tragically, sometimes they are. Such was the case in Laredo TX in November 2021, and details are below. Among the establishments where forklifts are heavily used are warehouses for big box stores such as Walmart, Amazon, Home Depot, and Lowe’s, none of which are likely Texas workers’ compensation participants.

The U.S. Department of Labor’s Occupational Safety and Health Administration (OSHA) reports that on November 12, 2021, a Texas forklift-related fatality occurred in Laredo, Texas. The employer’s name is Gogo Forklift Service, Inc. OSHA conducted an inspection after the fatality at 5002 Riverside Drive, Laredo TX 78041. The NAICS for Gogo Forklift Service is 811310, which is the Commercial and Industrial Machinery and Equipment (except Automotive and Electronic) Repair and Maintenance industry.

At this point, OSHA briefly summarizes the fatal Texas work accident under the “Hazard Description” in “Reports of Fatalities for CY17-21.” There is a note there indicating that the information is not comprehensive, and the data will be updated when it becomes available. The summary simply states the following:

  • David Martinez (50): fatally struck by forklift.
